Common Orthodontic Emergencies and What to Do Before the Next Appointment
From a poking wire and general discomfort to a loose bracket and more, a lot can happen when you're in orthodontic treatment. How do we know? We've seen it all — just about any kind of orthodontic emergency you can think of.
So, while these common orthodontic emergencies can feel like a true orthodontic emergency when it's your first time experiencing them as a patient, we can help you get through it. We'll walk you through some of the most common, and just know you can call us at any time for help.
Inconvenience or Emergency: What's the Difference?
Reality is, most issues are minor inconveniences that can usually be managed at home until you’re able to see us:
- A loose bracket or a wire that’s poking your cheek
- Mild soreness after an adjustment
- A lost elastic band
These common situations are typically not urgent, but it’s still a good idea to let the team at Shenandoah Valley Orthodontics know what’s going on.
So, what's a true orthodontic emergency? It's much more serious:
- Severe or unmanageable pain
- Injury to the mouth or face
- Signs of infection, such as significant swelling or bleeding
The good news is that true orthodontic emergencies are very rare. If you’re ever unsure, don’t panic—just reach out to us in Winchester or Front Royal for guidance and next steps.
Managing Discomfort During Orthodontic Treatment
Discomfort With Braces and Orthodontic Treatment
During the first week after your braces are placed and after routine adjustments, you may feel some soreness or discomfort. This is normal, and you do not need to worry. You may want to take acetaminophen or another non-aspirin pain reliever if your teeth are sore after adjustment appointments. You may also wish to use a warm washcloth or heating pad to reduce soreness in the jaws.
If you experience these or any other types of orthodontic emergencies, we encourage you to call our friendly orthodontists and team as soon as possible. We are here to provide the care you need!
How to Handle Common Orthodontic Emergencies
Knowing how to handle small problems at home can save you discomfort and help prevent bigger issues later. Take a look at this guide to common orthodontic emergencies, what causes them, and what you can do until you’re able to see your orthodontist at our Shenandoah Valley Orthodontics office in Winchester or Front Royal.
Loose or Broken Brackets
Brackets can sometimes come loose if you eat hard or sticky foods, or if your braces are bumped during an accident. A loose bracket may feel wobbly or slide along the wire, making chewing uncomfortable.
What to do:
- Cover the bracket with orthodontic wax to prevent irritation
- Avoid chewing on that side of your mouth
- Call Shenandoah Valley Orthodontics to schedule a repair as soon as possible
Delaying repairs can slow your treatment progress. Knowing when a quick fix is needed versus waiting for your next regular adjustment can help keep your treatment on track.
Poking or Loose Wires
If a wire comes loose, it may poke your cheek, lips, or gums, causing irritation or small sores. This is one of the most common (and uncomfortable) orthodontic issues.
What to do:
- Apply orthodontic wax to the sharp end
- Gently push the wire back into place using a pencil eraser or cotton swab
- If the wire is very long and painful, carefully trim it with clean nail clippers
Even if you manage a temporary fix, it’s important to contact your orthodontist so the wire can be properly adjusted and your braces continue working as intended.
Lost or Broken Retainers or Aligners
Retainers and clear aligners play a crucial role in keeping your teeth in the correct position. Losing or breaking one can be stressful — but quick action helps prevent setbacks.
What to do:
- If you wear aligners, put in your previous set to help hold tooth position
- Save any broken pieces to show your orthodontist
- Call Shenandoah Valley Orthodontics promptly for next steps
Even a short gap without your retainer or aligner can allow teeth to shift, so don’t wait too long to reach out
Soreness and General Discomfort
It’s normal to feel soreness after getting braces, switching aligners, or following an adjustment appointment.
What to do:
- Rinse with warm salt water
- Use mild over-the-counter pain relievers if needed
- Stick to soft foods like soups, yogurt, and mashed potatoes
This type of soreness usually fades within a few days as your teeth adjust. If discomfort lingers or worsens, mention it at your next visit.
Mouth Sores or Irritation
Braces may rub against the inside of your cheeks or lips, especially early in treatment, leading to small but painful sores.
What to do:
- Apply orthodontic wax over brackets or wires causing irritation
- Use an over-the-counter oral gel to numb sore areas
- Rinse with warm salt water to encourage healing
These sores typically heal on their own, but if irritation keeps returning, your orthodontist can help make adjustments.
Trauma to the Mouth or Teeth with Braces
Accidents from sports, falls, or other impacts can be more concerning when braces are involved. You may notice bleeding, swelling, or damaged appliances.
What to do:
- Control bleeding by applying gentle pressure with clean gauze
- Use a cold compress to reduce swelling
- Save any loose brackets or wires if possible
- Seek immediate dental or orthodontic care
In these situations, emergency visits may be necessary in addition to routine checkups.
Preventing Orthodontic Emergencies
A few simple habits can go a long way toward keeping your orthodontic treatment smooth and stress-free.
Watch What You Eat
Avoid sticky, hard, or crunchy foods like popcorn, chewing gum, and hard candy, as these can break brackets or bend wires. Softer foods are always safer for braces.
Protect Your Smile During Sports
If you play contact sports, wear a mouthguard to protect your teeth and braces from sudden impacts.
Keep Up with Oral Hygiene
Braces can trap food easily. Brushing and flossing regularly helps prevent cavities, gum irritation, and delays in treatment.
Don’t Ignore Small Issues
If something feels off—like a loose wire or bracket—contact your orthodontist early. Small fixes are easier (and more comfortable) than waiting.

Shenandoah Valley Orthodontics is absolutely amazing!!! From the very first visit the staff and doctors are amazing, kind and walk you through the treatment and process step by step. I would suggest everyone go to them!
Our experience with Shenandoah Valley Orthodontics has been truly memorable. The Invisalign journey wasn't always easy, and there were definitely some challenging moments along the way. However, the team was supportive, professional, and encouraging throughout the entire process. Over time, wearing Invisalign became second nature—my daughter often forgot she even had them on. The results have been well worth it, and we're very grateful for the excellent care and attention we received. Highly recommend Shenandoah Valley Orthodontics to anyone considering orthodontic treatment!
Excellent staff, excellent service! As an adult, I wanted to get my teeth in the best shape possible as I age. I love the Front Royal office - everything is clean and sanitary, the staff is friendly and knowledgeable, questions are answered throughly, and I felt comfortable every step of the way.
